Dabigatran appears to be a safe and effective alternative to warfarin. However, the drug itself is much more expensive ($400/month for 60 of the 110 mg capsules, which is a lower dose than used in the study, at one Canadian pharmacy's Web site). Some of that increased cost will be ameliorated because international normalized ratio (INR) testing is not needed. Before dabigatran is widely adopted, an appropriate cost-effectiveness analysis must be performed. (LOE = 1b)

Short-term study so dabigatran would be suitable for those we would have previously treated for 6-months post VTE. Longer term data are needed for those who have an indication for longer term anticoagulation (this will be the REMEDY trial - data collection finished ~ April 2011).
